A parent or legal guardian must accompany children under the age of 19. If a parent/legal guardant is not able to come, they must give verbal consent.
If you are unsure of what vaccinations your child is due for, please bring a copy of your childs immunization record. If you are unable to find one, contact your doctors office.
The recommended administration free per uninsured child for immunizations is $15.00.
